Job summary
We are looking to recruit experienced, caring, compassionate and committed ACPs who, acting within their professional boundaries, will provide care for patients presenting with undifferentiated, undiagnosed problems from initial history taking, clinical assessment, diagnosis, requesting of diagnostics and creating a management plan. They will demonstrate safe, competent clinical decision-making and expert care.
Positions are available for experienced ACPS in our UTC based at St Peter’s Hospital and/or the 3 GP acute illness clinics in Chertsey, Ashford and Woking.
The post holder will communicate and work collaboratively with all members of the multi-professional team to meet the needs of patients, and work to improve the quality and safety of care offered to the registered patients in the locality of North West Surrey.
Main duties of the job
Various Shift Patterns available:
- The Acute Illness Clinic is open Monday to Friday
- UTC covering 7 days a week.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
- You will assess, diagnose, plan and implement treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- You will utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
- You will recognise and work within your own competence and professional code of conduct as regulated by the appropriate governing body.
- You will take responsibility for your learning and performance, participating in clinical supervision and in yearly appraisal, personal development plans and revalidation.
- You will manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
- You will attend mandatory training courses and take responsibility for your professional development, undertaking learning as appropriate.

Job description
Clinical Practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care according to need
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and by evidence-based practice and national and practice protocols, and within the scope of practice
- Work with patients to support compliance with and adherence to prescribed treatments
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects, and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
- Support and manage the health needs of women presenting for family planning or sexual health consultation
- Meet the needs of patients presenting for opportunistic wound care
- Undertake minor surgery as appropriate to competence
- Communications
- Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
Delivering a Quality Service
- Recognise and work within own competence and professional code of conduct as regulated by the appropriate governing body
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures
- Deliver care according to NSF, NICE guidelines, and evidence-based care
- Work with others on improving the quality of health care by applying NICS policies, standards and guidance and national policies and initiatives as appropriate.
- Assess the effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation, including clinical audits, supporting the implementation of improvements where required
- Evaluate patients’ response to health care provision and the effectiveness of care
- Support investigation reviews and management, and identify learning from patient complaints, clinical incidents, and near-miss events to be shared across the service and wider organisation.
- Participate in the ongoing maintenance of quality governance systems and processes across the organisation and its activities
- Work with Lead Nurse to ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Provide guidance and support as appropriate.

- Apply infection-control measures according to local and national guidelines
Learning and development:
- Attend mandatory training courses and take responsibility for own professional development, undertaking learning as appropriate
- Participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development, including revalidation
- Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning and sharing good practice.
- Other responsibilities include (this list is not exhaustive):
- Management of long-term conditions: involvement, implementing and using protocols and clinical guidelines by providing leadership and mentorship to the Advanced Nurse Practitioner if within the scope of practice.

- If needed, lead in Infection Prevention and Control (IPC) within the service
- Support the fulfilment of the Quality Outcomes Framework and local Quality Improvement Schemes.

Person Specification
Qualifications
Essential
- Qualified Nurse/ Paramedic with Master’s Level Advanced Clinical Practice or equivalent via Credential/portfolio route.
- Independent Prescriber
- Experience in UTC/GP setting (dependent on site of preference)
- Evidence of continuing professional development/ appropriate mandatory training
- Able to demonstrate clinical knowledge, skills, and expertise
- Ability to work independently to provide patient-focused care.
- Self-motivated team player
- Tolerant, tactful, and co-operative with the ability to communicate effectively in challenging situations
- Values align with NICS

- IT computer experience (Emis, Cerner)

Experience
Essential
- Evidence of working as a Clinical Practitioner with skills and knowledge of working in a GP practice as an ACP. Nurse-led management of minor illnesses, minor ailments, and injuries. Nurse-led triage. Minimum of 2 years working at Band 5 or equivalent with experience in urgent or emergency care is desirable.
- Experience in chronic disease management. Experience with EMIS is essential
- Demonstrate commitment and ability to support staff teaching, research, and education.
- Demonstrates the ability to effectively manage resources. Demonstrate experience within the strategic planning process.
- Computer literate Patient patient-focused.
- Ability to work independently and to specific deliverables. Interest in primary care services, workforce and at-scale provision. Ability to work within own scope of practice and understanding when to refer to GPs. Able to demonstrate clinical knowledge, skills, and expertise.
Desirable
- Working in the local area designing/ developing new services, Clinical Supervision IT, Clinical system, Experience in Resource Management, Experience in audit and research, Knowledge of primary and urgent care pathways, Familiarity with commissioning pathways, Knowledge of local services
