Surrey and Borders is looking for someone to support the delivery of the Trust’s Patient Safety Incident Response Plan by undertaking and contributing to high quality patient safety incident reviews, ensuring that a proportionate response is taken in order to identify and embed learning from patient safety incidents across the organization.

Main duties of the job

Support the delivery of the Trust’s Patient Safety Incident Response Plan (PSIRP) by undertaking and contributing to high-quality patient safety incident reviews.

Ensure that a proportionate response is taken to identify and embed learning from patient safety incidents across the organization.

Be responsible for the day-to-day management of patient safety incident reviews as allocated by the Head of the Safety Team.

Undertake reviews in line with a systems learning approach, ensuring involvement of patients, carers/families, Trust staff, and where appropriate, third parties in the review process.

Detailed job description and main responsibilities

Work collaboratively with internal and external stakeholders to review, analyze, and use judgement in collating evidence and findings using the agreed PSIRP methods.

Identify systems learning from patient safety incidents to guide sustainable risk reduction and drive service improvement.

Ensure that Just and Learning Culture principles and human factors concepts are embedded throughout the review process.

Analyze and interpret complex and sensitive clinical and safety situations, using advanced communication and coaching skills to facilitate learning and improvement.

Contribute to decision-making processes that support patient safety, quality improvement, and organizational learning.

Qualifications
Essential criteria
- Registered mental health nurse and vast experience in leadership, investigations and healthcare delivery
- Trained in SEIPS or willing to undertake training in the SEIPS approach
Experience
Essential criteria
- Minimum 2 years in extensive risk and Safety experience
- A working knowledge of mental health legislation including the Mental Health Act, Mental capacity act and safeguarding requirements
- Computer literate especially Microsoft Word, Excel and PowerPoint.
