We are looking for a Support Worker to join our Hospital Discharge Service. You will provide a point of contact for vulnerable people admitted to hospital, ensuring that effective support is provided from admission to discharge. You will work alongside external agencies such as statutory and voluntary services to facilitate access to suitable accommodation. The Hospital Discharge Team works with people from critical care and primary care, providing support with housing and identifying services for any ongoing needs.
The ideal candidate will be flexible in their approach, able to work on their own initiative, a natural problem solver whilst being responsive and compassionate towards the people we work along side. .
Experience is preferred but not essential as full training will be given.
