We are looking for a Physical Health Lead Physical Health Lead (RGN / RN-A). with a passion for delivering outstanding care.
You’ll be working 38.5 hours a week, at Blackheath Hospital.
Cygnet Hospital Blackheath provides inpatient low secure and PICU services for men with a mental health illness.
We accept urgent admissions from all over the country for people in acute psychiatric distress and can assess and transfer urgent prison referrals swiftly. We also provide an intensive programme of recovery in our low secure mental health ward, developing a personal therapeutic timetable to meet the needs of service users whatever their stage of recovery.

Your day-to-day…
Clinical Practice:
- Assess, plan, develop, implement and evaluate programmes to promote health and well – being, and prevent adverse effects on health and well – being.
- Overseeing and evaluating individual care and treatment plans for patients with physical health conditions
- Identify and manage as appropriate, care and treatment plans for patients at risk of developing physical health conditions
- Regular monitoring and interventions for patients with long term conditions such as diabetes, asthma, hypertension, epilepsy etc
- Prioritise physical health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self – care.
- Provide information and advice on physical health medication regimes, side effects and interactions.
- Assessment and management of patients with wounds
- Work closely with the visiting GP overseeing GP referrals, annual physical health check referrals, referrals to specialist services etc
- Working alongside the ward Dr’s with phlebotomy and ECG services at Cygnet Hospital….. making sure all are done accurately and timely.
- Responsible for physical health induction and training for all staff
- Oversee referrals to dieticians, opticians, dentists, hearing tests, chiropody and SALT
Regularly check physical health supplies/equipment and order as required

An experienced RGN/RN-A with a current Pin Number, you’ll have…

- The ability to maintain documentation, undertake audits & demonstrate integrity in your role
- Gread leadership skills
- Enthusiasm to develop the Practice Nursing role from an evidence based perspective
- A good understanding of the latest nursing practices
- A commitment to patient recovery in order to effectively monitor, manage & reduce risk
- Compassion, resilience & a dedication to empower & support service user independence
- Essentially have knowledge of patients with long term conditions, health promotion strategies, and clinical governance issues in primary care and communication skills that maintains a pleasant and professional relationship with patients and colleagues.
Desirable:
- The ability to deputise in a clinical lead role
- An interest in leadership, research & assessment
